MEDICAL SCHEDULING FORM

This form is for CURRENT FOSTERS. 

Please complete this Google form for any vetting that is needed for your foster pup. The medical team will review, make your appointment and reach back out. 
 

TCAP Vetting:
If you plan to use a TCAP location for vetting, you must submit a Medical Scheduling Form and note the specific TCAP location prior to your visit. 

This allows our Medical Director to issue a prepaid certificate in advance. Visits must be approved and prepaid beforehand, as appointments made without a certificate cannot be reimbursed or covered by NTASR.
